What information do I submit when I upload a provider?

When you upload a provider for approval, the app will ask you for the following:

• Provider's first name
• Provider's last name
• Provider's email address (please double check for accuracy!)
• Provider's cell phone number
• Provider's relationship to you (mother, father-in-law, friend, sitter, etc.
• Provider's date of birth (make sure to close the calendar after you enter this!)

It will also ask you:

• The zip code you will primarily be booking your provider in.
• The hourly rate of your provider (please agree upon a rate prior to uploading them. The app will let you know what your hourly copay is once you enter your provider's rate).
