What information do I submit when I upload a provider?
When you upload a provider for approval, the app will ask you for the following:
- Provider's first name
- Provider's last name
- Provider's email address (please double check for accuracy!)
- Provider's cell phone number
- Provider's relationship to you (mother, father-in-law, friend, sitter, etc.
- Provider's date of birth (make sure to close the calendar after you enter this!)
It will also ask you:
- The zip code you will primarily be booking your provider in.
- The hourly rate of your provider (please agree upon a rate prior to uploading them. The app will let you know what your hourly copay is once you enter your provider's rate).
